What is the IMAT exam
IMAT (International Medical Admissions Test) is administered by Cambridge Assessment on behalf of the Italian Ministry of Education, Universities, and Research (MIUR). It is the universal entrance test for all 16 Italian public university English-medium Medicine and Surgery programs.
IMAT scores determine two things: whether a student qualifies for a university's cut-off in a given year, and which university they can access through the preference allocation system. A higher IMAT score opens more university options; a lower score may still qualify for less competitive institutions.
IMAT exam format
Duration and format
100 minutes. 60 multiple-choice questions. Each question has 5 options with 1 correct answer. No calculator is permitted. The exam is paper-based at registered test centres.
Scoring system
+1.5 marks for each correct answer. −0.4 marks for each incorrect answer. 0 marks for unanswered questions. Maximum possible score: 90 (all 60 questions correct). Negative marking means blind guessing reduces your score.

Exam date and registration
Held annually in mid-September. For 2025–26 intake the exam was September 17, 2024. The 2026–27 exam date is expected mid-September 2026. Registration opens several months before the exam — check MIUR and Cambridge Assessment websites for exact dates.
Where Indian students can sit IMAT
IMAT is conducted at over 20 countries globally. In India, the approved test centres are New Delhi and Chennai. Students can also sit the exam at their preferred Italian university if they are already in Italy, but most Indian applicants use one of the India-based centres.
Students must register for a specific test centre and cannot change it once confirmed. Registering early is important as test centre capacity is limited, particularly in India.
IMAT section-by-section preparation guide
Biology (23 questions)
The highest-weighted section. Core topics: cell biology and cellular processes, genetics and heredity, human anatomy and physiology (cardiovascular, respiratory, nervous, endocrine, digestive systems), microorganisms and immunity, ecology. Indian Class 12 biology covers most of this content — IMAT requires application and analysis-level answers, not just definitions.
Chemistry (15 questions)
Covers atomic structure, periodic table, chemical bonding, stoichiometry, acid-base chemistry, oxidation-reduction, thermochemistry, organic chemistry basics. PCM background covers the inorganic and physical chemistry well. Organic chemistry questions require IMAT-specific preparation as the depth differs from Indian boards.
Physics and Mathematics (13 questions)
Physics: mechanics, thermodynamics, waves, optics, electricity, magnetism. Mathematics: algebra, functions, logarithms, geometry. Indian NEET and JEE preparation covers physics strongly. The mathematics component is lighter than JEE but requires specific IMAT-style practice.
Logical Reasoning (5 questions)
The most underestimated section for Indian students. Types: deductive argument analysis, inductive generalisation, critical evaluation, pattern recognition. There is no standard Indian board preparation for this. Dedicated LR practice using IMAT past papers is essential. Losing marks here unnecessarily is the most common avoidable IMAT mistake.
Reading and General Knowledge (4 questions)
Based on a short English passage. Tests reading comprehension, inference, and critical evaluation. Strong English reading skills are sufficient. No specific preparation other than practising passage-based MCQ reading is needed.
Competitive IMAT score ranges by university
IMAT cut-offs shift year by year based on the applicant pool. The figures below are indicative based on recent cycles and should not be treated as guaranteed thresholds. Scores are out of 90.
Highly competitive (score 80+)
University of Bologna, Sapienza University of Rome, University of Milan. These universities draw the highest-scoring IMAT applicants globally.
Competitive (score 65–80)
University of Padova, University of Pavia, University of Turin, University of Milan-Bicocca, University of Rome Tor Vergata.
Accessible (score 50–65)
University of Bari, University of Parma, University of Naples Federico II, University of Campania Vanvitelli, University of Messina, Marche Polytechnic University.
Lower entry threshold (score 40–55)
University of Catania, University of Cagliari (newer program). Suitable for students who need more IMAT preparation cycles.
How to prepare for IMAT as an Indian student
Start preparation 6–9 months before the September exam date. A 3-month preparation window is sufficient only for students who are already strong across all 5 sections.
Use official IMAT past papers (available from Cambridge Assessment and MIUR). Do at least 5–6 full timed past paper sittings before the actual exam.
Do not neglect logical reasoning. Most Indian students spend 90% of preparation time on biology and chemistry — and then lose 3–5 marks on LR through insufficient practice.
Negative marking is real. Do not attempt a question unless you can eliminate at least 2 of the 5 options. Random guessing across 10 questions costs you 4 marks on net even if you happen to get 3 right.

What happens after IMAT
After IMAT results are published (typically within 3–4 weeks), students submit their university preference list. Allocation is based on score and preference order — higher scores get priority access to more competitive universities.

Is NEET required to appear for IMAT?
No. NEET is not required for IMAT registration or Italian university admission. IMAT is the only entrance requirement. However, we advise Indian students to sit NEET if they want to preserve their India-return pathway.

How many times can I sit IMAT?
There is no limit on the number of IMAT attempts. Students may sit IMAT in consecutive years and submit their best score for the university preference round.
